Un nuevo panorama en el tratamiento del Cáncer de Pulmón
Resumen
El Cáncer de Pulmón (CP) es un problema importante de salud pública, en nuestro país, se registraron 7,588 casos nuevos y 7,100 muertes durante el año 2020, siendo la cuarta causa de mortalidad por cáncer. Este se origina cuando algunas células del pulmón, sufren cambios genéticos y epigenéticos (modificaciones que no alteran la secuencia del ADN) que les permiten dividirse de manera descontrolada, invadir tejidos normales y en ocasiones, migrar a otras partes del cuerpo, donde pueden formar nuevos tumores.
El concepto de cáncer de pulmón como una enfermedad sumamente maligna y agresiva, es una visión compartida por un gran porcentaje de la población. Sin embargo, los avances tecnológicos en las áreas de aplicación de la biología molecular y la biotecnología han permitido, determinar algunas de las causas moleculares asociadas al CP y desarrollar fármacos que vayan dirigidos hacia moléculas específicas (“blancos moleculares”) involucradas en la respuesta a la terapia, revolucionando los esquemas de tratamiento de dicha neoplasia y ampliando la espectativa de vida de los pacientes. Entre las que se encuentran el receptor de crecimiento epidermico, EGFR, su activación promueve la proliferación de células normales;aunque en algunos tipos de CP este receptor se encuentra sobre-expresado o activado constitutivamente, lo cual favorece el crecimiento y la propagación de las células tumorales. Dos tipos de fármacos han sido desarrollados con la idea de inhibir el papel de dicho receptor en el CP, los inhibidores de TKI y los anticuerpos monoclonales, ambos constituyen uno de los mejores ejemplos de moleculas útiles en las terapias personalizadas, con resultados alentarores.
A la fecha, este tipo de terapias han incrementado la supervivencia y la calidad de vida de un gran número de pacientes, abriendo por primera vez, la posibilidad de una cura en pacientes con enfermedad metastásica. El desarrollo contínuo de nuevos fármacos dirigidos, brinda la esperanza de que cada vez, un mayor número de pacientes puedan recibir tratamientos exitosos.
